If you've ever had an accident at a company site (workplace, shop, training unit etc.) you may not have thought to fill in an accident record. You may have left the site/shop but wish you'd completed an accident record - well now you can.
In the event of a serious accident you can then follow up to make sure they report it to HSE and you may want a record for an insurer to make a claim.
It is easy to use. You simply fill in the form, click to give consent, sign and then email it to the company. You will have a copy in your sent items.
